Your proposal should look something like this:

1)Thanks for the opportunity
2)Your understanding of the job
3)How you would complete the job
4)Why your company is the best for the job
5)Your price - with breakdowns if appropriate
6)Any "must haves" assumptions made etc in getting to the price
7)Last thanks and way forward.

Item 5 and 6 should be on their own page so that they can be removed if necessary.

Put your company details and contact details on the header of each page.
Put your copyrights, date and page number of number of pages on the footer.

Include brief cover letter - with
1)contact details
2)who is their contact for this bid
3)thanks for the opportunity
4)very brief overview of bid - no price
5)time frame that bid is current
6)thanks and hope to hear from you soon.

A professional proposal includes:
A cover letter - a simple about you (what makes you better)
A Statement of Work - describe what you're doing
Terms of Payment - How much and how you expect to get paid (2% 10, net 30 is common in your industry, so is "upon completion")
References - optional if you dont wish to include
