If you really want to improve your English, the best way would be to spend some time in an English speaking country. If that's not possible, you could try listening to radio, reading books/newspapers, watching TV shows in English etc.

Your English is not really that bad. I know lots of native speakers who can't even string a coherent sentence together. Don't worry so much about vocabulary. To sound fluent in English it is better to concentrate on grammar and common usage. Don't get so hung up on using long words when simple English is clearer and better.

I note your use of words such as "initiate" when you mean "start" and "impressive" when you just mean "good". "Advice" is singular/plural - an uncountable noun - and doesn't need "an" before it - it's just "good advice" that you are looking for.

Also it would simpler and be better to say: ". . . my writing and grammar skills are terrible", "I am so weak in these areas that I am worried.", "I am really desperate to improve my English in some way, whether it be listening, speaking or writing ", "so can anyone give me some good advice please?".
