Read, and take an etymology course. I have done the former, but not the latter. I regret not taking etymology. It will be like the most important class for you during ACT/SAT(unless your not in high school anymore, but you get the idea). First buy a copy of "elements of Style" by William StrunkReview the definition of a sentence. Review the parts of a sentence.
Then study grammar and punctuation. . READ good literature, learn sentence structure and how to analyze what makes a good sentence.
The above is the easy part. The hard part is writing. Practice writing. Assign yourself paragraphs on topics and write one page essays over and over until you can convey what you are saying clearly and succinctly
Don't rely on either spell check or grammar check on the computer. They are not reliable for upper level English. Invest in a good dictionary.
Read and write. It is that simple and that hard.

Like you, I'm a crazy reader.But because I want to improve my writing, I take notes, sometimes in my head, sometimes actual, written-in-ink notes. If I ever come across a word that I either don't know or know, but always forget, I jot it down in my Word Book (Yes, I have a book of words. Yes, I am a dork. XD). Then, after I'm done reading, I look up the definition in the dictionary. I've learned so many fantastic words this way.
Another suggestion is just to write. You'll see the fastest results this way, too. Have you ever looked back on a writing piece that you'd written only a few months before, and say: "This is quite possibly the worst thing I've ever had the misfortune to read"? This is how I *know* that I've improved. And it always feels great to know that.
I like to read books about writing, too. I've found Write Great Fiction (it's a bunch of books that help with certain points of writing like characters and plots) to be really helpful. http://www.amazon.com/s/ref=nb_sb_ss_c_1…
