Something relevant to the position would be fine but it doesn't need to be anything new. They are most likely just looking for your writing style, use of proper grammar, punctuation, etc. Make sure what ever you submit is typo and error free.

It depends on what kind of job you are applying. To apply for a news reporter's job, it is what you have written in the past for another newspaper. To apply for a PR job, it is the PR piece you wrote before. If it is a technical writer's job, it could be a technical manual or instruction you wrote in the past.

Well, it never hurts to suck up a little, maybe you could write about why you want the job, or what would make you a good candidate. Just be sure to keep it simple, and don't get too sappy with it.
