A cover letter is the body of the email is fine. Take the time and learn who to address it to. Often when I see a resume and it's addressed to "To Whom It May Concern" i get the impression that it is a canned resume. It is best to address it to the person doing the hiring or selection process.

Your emailed cover letter needs no date as it is redundant. The received date is enough. Your email address is fine and no other address is required if you have the address in your attached resume.

No, employers will often input your information into a database of some sort, and will prefer to have your cover letter and resume separate. As the others said, references don't need to be included.

It is generally more professional and polite to include a cover letter...nothing fancy, just a brief outline of why you'd like a job and personal attributes. Staple your cover letter to your resume, and make sure they are both one page only as it can be annoying for employers to have to sift through pages and pages...
make it look a little bit different so it stands out, like underlining your name in coloured font but make sure it still looks professional.
go into the stores you consider working at and ask to hand it in to the manager. Sometimes stores have a specific written application form or request that you apply online, so remember to ask if they don't accept your resume!
