3 things.....Sometimes yes companies can right to the reporting agency to correct something...like a mortgage... instead of a shortsale. 'settled for less then owed' you can ask the mortgage company to report 'paid in full'
2nd thing....there is the 'fair credit reporting act' which basically makes it so you can just about get any inquiry off your credit if you know what you are doing.
3rd...best route might be to hire a credit repair company..there are good ones out there, shouldn't cost more then 1k to have this done depending how bad you are damaged.
The biggest misconception on credit is that it can't be repaired...you just have to have the tools to do it.

your instructor should have handed out a paper prompt witht the requirements on it.in general -- an outline covers your major points (I II III) in one or two words, with lesser points (A B C) underheath (as in ! (next line, indented a little more) A) all I, II, III s line up with each other, all A B Cs line up with each othern and are more indented than the I, II, III points. you can go into further detail, using (first) 1, 2, 3 and then a, b, c
they alternate, so you'd have (one line) I (roman numerals)

next line 1
next line a
each line is indented about five spaces further than the previous. anything after the first two is probably too detailed unless your paper is over 20 pages and very detailed
a table of contents is simply a listing of your chapters (or major points in the paper and what page they're on. it also lists the index (if there is one) (endnotes) if there are any) and the introduction (if there is one, which will be numbered in lower-case roman numerals. your regular page numbers will start at page 1)
sources are the places where you got your information. they are the footnotes or the end notes to the paper. each one will include enough informtation that anyone reading your paper could get that same source and find that same tit-bit of information. if your instructor wants these you should have received a paper prompt that would indicate that, and the form that the instructor wants you to use. in doubt, google on, and use the MLA standard.
a template usually refers to the way your instructor wants the paper laid out. they may want your name, the course name and number, the nstructor's name, and the date (or, at my University,Winter Quarter, 2009) plus whatever information they wanted (like page 2 of 20) and the margins they wanted (generally 1") as well as line spacing (usually double, except for the footnotes and the headers)
honestly, if the instructor wants that much detail, there needs to be a prompt paper given out. and, just in case your're a stick-it-wherever-and-forget-it guy, you need to start changing your habits. when you get in the Real World, they don't want to hear that the dog ate your copy of the annual report, or the schematics, or whatever.
