We Answered:

Yes, passive voice and past tense, and very formal language. The way I get my students to remember it is to use the nursery rhyme "Jack & Jill".
Jack and Jill went up the hill
To fetch a pail of water
Jack fell down and broke his crown
And Jill came tumbling after.

In passive voice...

The hill was ascended by Jack in the company of Jill.
The purpose of the ascent was the procurement of a pail of water.
A precipitous and rapid descent of the hill was undertaken by Jack, resulting in severe damage to the cranium. A similar precipitious descent was subsequently effected by Jill.

Ludicrous, isn't it? But it sticks in their heads!
