There is no standard abbreviation. My department used DR for the standard Death Report, but I found that the standard police word for a murder report is Homicide. When the death is ruled to have been caused by unknown person(s), it then becomes a homicide. Later reports would be titled depending on the degree of offense (i.e.: Capital Murder).

A good lab report consists of the following sections, properly laid out.

Cover page: should contain name, lab name, course, student ID, lab partners (if applicable) and name of lab demonstrator.

Aims and objectives: what you set out to do in the lab. This includes practical things, such as making aspirin, or analysing vitamin C, as well as the background, such as learning about certain reactions, or how to apply electrochemistry.

Introduction: a theoretical background on the experiment. Do not hypothesize what could happen, but stick to known facts, such as the reactions taking place. Write about the background of a reaction or an analysis, the reaction mechanism and how it applies to real life things, or about how the analysis will work, by giving equations and describing how the practical side works.

Methodology: in most cases, "As per lab manual" is accepted. Otherwise, list the steps you do.

Results and calculations: tabulate them neatly, and show an example of each calculation type you do. Include graphs in this section. Raw data should go in a table, calculations and graphs should be presented neatly.

Discussion: Go through the results, and explain them to the reader. State, with reasons, whether results are acceptable or not, and interpret them in terms of the objectives. Start explaining results, compare and contrast with the theory, provide sources of error etc.

Conclusions: a quick summary of what you have concluded from the results. No need for high detail, that is what the discussion is for.

Bibliography: if you used books or other resources, state them in a separate section, and provide the points where you used them with clear markings.
