Ohhhh APA...they're so anal. About EVERYTHING. Abstract has a whole page to itself, yes. Then on the next page, before you start your intro, you put the title of your paper centered on the first line (not underlined or italicized or anything, just centered in normal font. Then the next line you start the intro.

It depends on your professor. According to APA, it is suggested that you write in parenthesis the author's last name and publishing year at the beginning of each citing, followed by the page number at the end of the citing. Some professors just require footnotes. To be on the safe side, I'd just cite each time. Who needs the hassle of being accused of plagiarism if it can be avoided? Good Luck!
