Well the proper way to do it is to put it in italics. But when people hand write something, italics are hard to make, so that's where the underlining came from.

Well, the actual format depends on the system you are using (APA, MLA, etc...) but you would cite every time you did not use your own information. If you need to cite 5 times, then so be it.

You need to check the rules of the type of citation system you are using. For APA, for example, if you use a single author more than once, you don't have to put the date every time. If you use a quote, you MUST include the page, etc.....

Basically, your work might look like this for the author J. Doe and F. Smith who wrote a book in 2007 (this is APA, though).

Lorem ipsum dolor sit amet, consectetuer adipiscing elit. In mattis (Doe & Smith, 2007). Donec blandit. Mauris dolor erat, dapibus at, suscipit ....

Doe & Smith (2007) suggest vestibulum lacus arcu, aliquet ut, facilisis ....
