Ask your teacher for help.

For background info you need to interview the parent about medical history, developmental milestones (i.e. if they were met on time or not), current medication, living situation, student's likes/dislikes, and behavioral problems at home if any.

For observations: you need to observe the student and just write everything you see within that time frame. You do not need to analyze or interpret what/why the student is doing what he/she is doing. For example: During classroom instruction, student was observed to run around the classroom and hit everyone near him with his shoe. Teacher asked him to sit. Student did not comply and began yelling the national anthem while hopping on one foot.

For templates on the KABC-II, look at the manual for a good starting point.
