Well, you have many ways to write a science report and depends on what you want it to have and sometimes, on the teacher you have as well (some are very picky with how a report has to look and its contents)

http://chemistry.about.com/od/sciencefai… this one looks good (although its missing some parts), you can add the discussion to the conclusion part or put it right before the conclusion and the same to the data and results (you can put it together or separate).

This one looks a bit more complete http://www.monash.edu.au/lls/llonline/wr… (appendices are usually graphics, schematics, tables and/or pictures you think are important to the experience (and report) in question) and its how I do mine (university level). You don't have to put an abstract although it looks cool and professional :P
