Something along the lines of "Respectfully submitting my 2 week notice as another opportunity has arisen." I doubt many fast food restaurants are really given the courtesy of a 2 week notice, so anything you jot down will probably be sufficient.

Do not write a letter. Get a proper legal "Pay or Quit" or "3-Day Notice" form for your state. You can often find the forms at Staples, etc. Or you can find your state's legal requirements regarding these forms, how they must served, etc, in landlord-tenant law. Your reference librarian will probably be familiar with what books are available for your state's landlord-tenant law (e.g. Nolo Press guides) without having to go to a law library and wade through the actual law books or get an attorney.

If you mess up the form, or service of process, you're in trouble and have to start over from scratch.

The section you note in parentheses should be very clear - you describe the lease, dated when, signed by who & who, signed where. You can also put "See Attached Copy of Lease" and attach a copy of the signed lease. You are supposed to have the signed original, and you are going to need that.
