Its the thought that counts. really. Is ther a particular piece of music tha is special - something they made you play so much it became a joke? Something that was particularly insightful or that you shared together? God out and get that sheet music from a music store, Write on it some personal thing like "I played this so many times I'l know I'll never forget it or you for teaching it to me the right way. Thank you so much!" Then frame it and give it to them. "Or, this is the music of God and It will always make me remember How He sent me such a wonderful teacher." Or some other special thing. It it's "As time goes by" you could write "We'll always have the practice room." Make it appropriate to what you feel for the teacher.

Don't! You're letter would be a week late so skip it. Call them up personally and say thank you. Use your own language and reconnect with them or you're sunk. If you send them a letter they will think you're really slow with follow-up. (Bad for a commission based sales job) After you call and talk with them, send them a quick thank you e-mail summing up what's next and your interest. Try to schedule a follow up meeting in your call to close the deal.

I have to say - a handwritten note would mean a lot more to me - even though I consider myself a pretty techie person. There is something impersonal about an email that is off putting.

As for making each message personal...
Make reference to something that you have learned from them, or something you did with that teacher (a trip, a guest speaker etc.) that made an impact on you. If they have been personally supportive (not just academically) mention that. If they've influenced your decisions about your future, definitely tell them that.
