If you know the basics of writing a formal letter (address and date, name, salutation) then the rest is a matter of having a sentence or two explaining what the trip is, and where, and when. If there's a website or contact details, or another teacher is organising the trip, mention it here.

Then you need a simple sentence asking for permission to go, which could be as simple as 'Please may I have your permission to go'. Then you could add a reason why you think it's necessary, but this should be again, one single short sentence. If another teacher has said it would be useful to go, then you should mention this.

If you need a big, big explanation, then that should go first, along with the trip description. However, you shouldn't need a big, big explanation in most cases. The main point is to keep each point short and clear and to use good English.
Then you can add thanks and close the letter. For example :
