It sounds like you know why she's mad at you, so explicitly state that you are sorry for what you did. Do not make excuses or try to rationalize it, even if you really didn't mean it in a rude way. The time for that will be if she chooses to discuss what happened with you. You have to swallow a lot of pride in apologies, especially if you don't see what the big deal is.

Most importantly, KEEP IT SHORT. Your friend is upset with you, so she's not going to want to read a page-long letter from you. Make your point as soon as you can, say that you are willing to talk about it and hope she accepts your apology, and end the letter there.

Its your wedding-and weddings are expensive, save yourself the money you spent on stupid apology cards and invite them! If they mean that much to you, invite them, and if your not, don't bother with the card. I'm sure they'll still send you a card and present and be happy for you.
