I can guarantee your teacher will love it. If she cares enough to do that for you, she won't be expecting anything in return but a note will absolutely make her day. Things can sometimes be hard so say in person, because you're embarrassed or you just can't express yourself on the spot, and a note is the perfect solution for this! Write whatever your heart is telling you. Don't let your mind edit as you write by thinking 'oh that sounds lame' or 'she will this this is silly'. I promise you, if it is how you truly feel, then it can't be stupid.I have a teacher who I am really close to. She has taught me for 4 years and she's basically like my second mother. I've written her countless notes and cards. I know it is silly, but I just can't tell her to her face how much she means to me because I can't put what I feel into actual speech. She knows what I think of her though, I make sure of that with the notes. And she loves them. She has a letter I wrote to cheer her up pinned up on her office wall and she looks at it whenever she is upset.
Earlier this year I had another teacher who really helped me and I wanted to let her know I appreciated it. I wrote her a note, but I was really worried about how she'd react. I mean she is really nice and totally amazing but I thought she'd think it was silly. Her reaction was definitely the opposite though. She hugged me and told me I was a beautiful person.
Since then I realised that teachers are humans too and of course they'd love hearing how much their students admire them. I'm just about to finish year 12 and I've written a letter for each of my favourite teachers.
If you're still embarrassed you can hand her the letter and walk away or leave it on her desk. Good luck and I hope you get that personal stuff sorted out.

When I was job hunting, I had a loose-leaf notebook in which I kept copies of all correspondence with each company. If I was fortunate enough to get an interview, I always sent a prompt thank-you letter, and kept a copy so I knew I'd sent it. You'd be surprised at how many job-seekers omit this courtesy because they think they have no chance to get the job. Interviewers really appreciate it. One time my husband had to cancel an interview appointment because of some emergency. He phoned the man to let him know, and the man was amazed and gratified that my husband was polite enough to do so. He invited him to interview at a later time, and actually offered him a job.Such a letter can be brief. It should be cordial but businesslike. Here's one pattern:
