I used to work for multiple orgs and one of the easiest ways is to make a 2" column down the left side of the page and list your officers down that side. Its a good place to put your web site phone and other contact information to make it all look uniform.

First of all, if it is a donation, it should be for a cause, not just a holiday party. Or, is it for a raffle?

All you really need to send is a letter like this:

Dear Mr./Mrs. Jones,

Thank you for including <name of company> in your holiday celebration. We are happy to share this <item>/$100 with you.

Have a happy holiday season,

Very sincerely,

Your boss's name

Have your lawyer write it up. It should only cost you a couple hundred bucks at most and it will be well worth it if any problems arise.
