Typically, bad employees are not entitled to a letter of reference. I don't know if this is true for all states, but in my state you are only really allowed to tell someone that a person is not rehirable. Giving a description, even in funny wordplay which does anything else can get you sued. You might want to check into the situation in your state before you write your letter.

However, regardless of anything else, it is poor taste to write an ambivalent letter of reference. It is the employer's choice to write one or not write one and if someone was a bad employee than they should simply be told they will not be receiving a reference.

Ask now, well in front of the deadline. It it is likely a busy time of the year so their priorities may not be writing a letter so some lead time should increase the likelihood of a very positive letter before the next term gets into full swing. To add depth the letter, you can provide them with a personal statement or something else that highlights what you think is important to you in terms of your past performance and your future ambitions and he might incorporate these things into your letter.

Let them request it -- lots of times they send their own questionnaire out
to get filled in, and sometimes they call and actually talk to people. It will look like you're pushing it on them if you attach it. If they get the info from him by one of the other methods, he's still gonna be brimming w/praise about you, so don't worry...IF their ad SAYS to attach references, then go ahead. Otherwise, don't.

If you are able, you should stop by and see them in person. Say "I'm applying to Super School for Grad Program and I'd like you to write a letter or rec for me. Is that something you could do?" Take a copy of your transcript or resume. They should be able to write a letter without any help, but it certainly doesn't hurt to offer.

If you can't see them in person, call them and ask.

If you can't get them on the phone, I think email would actually be best. That way they can just hit reply to get back to you. Much easier.
