I do cover letter first (as it is a general intro to you and I always include in the cover letter " I am attaching both an app & a resume--so its silly to not have that first) then the formal job app, then the resume last. To me that makes sense--an intro, then the application followed by the more in-depth resume.

Letters of reference. Be sure to pick out 3 people who can vouch for your attributes and also who write intelligently. Your best friend might know you really well, but wouldn't be a good reference if they can't put it into words that will add to your position. If its submitted electronically, it is not easy to do. You would have to scan your signature to an image file and then import it.

Just type your full name and put it in Italics.
