Dear Sirs,

As you will understand my current leave of absence due to maternity reasons is scheduled to end on April 30th 2009. I am writing to you to request an extension of this leave by a further sixteen weeks, therefore returning to work during the week commencing 17th August 2009.

I would appreciate it if you could contact me regarding arrangements for this. I am available on telephone number ...... and would be able to attend a meeting with yourselves should this be more convenient.
