Contribution to the working environment (idiot or thinking ahead)
Mannerisms - behavior, courtesy
Ability to work at different levels of the job and how they worked with colleagues.
Would they work on to finish a project.
And generally what type of person they were like.
Be Honest and factual try and highlight the good points. If you want to refer to a bad point say "this person is capable of" this means they could, but they didnt.

There are a lot of questions here. Let's go through them.I doesn't matter what the salutation is. No admissions committee member is even going to read the salutation, it is completely irrelevant. I don't like the idea of putting a specific person's name on it, because it will not be looked at by a specific person. I looked at many rec letters and I would have been a bit put off had the letters been addressed to the head of admissions (had I looked at the salutations, which I did not). So put 'to whom it may concern' or 'admissions committee' or 'admissions office' or something like that, but don't worry about it.
The letter should be typed and signed. It would help if the letters were individualized for each school (so they say Harvard or Stanford somewhere on the letter), but that's not a big deal either. No photo copies, especially of the signature. You should not have access to the letter once you ask him/her to write it. So, if photo copies are made, you should certainly NOT be the one doing it.
You can have the letter sent separately to the admissions office, or you can collect it and send it with your application. Either way, it needs to be sealed in a way so the admissions office knows it was not read by you. Provide the person with envelopes and ask them to seal it. I would have the writer send it in and give them addressed and stamped envelopes for each school.
Your name obviously should appear on the letter. Most schools have forms for recommenders to fill out with their letter. Those forms will have all the needed information to link the letter with your application. Try to find these forms. Some of the form will be filled out by you (your name and such). If their is not a specific form for 'family friends' just use a teacher form. If your university does not have that, you will need to find out how that school links letters to an application and make sure that info is on the letter when it is sent (do they give you an application ID #?, etc). The admissions office needs to be able to link and combine it with the rest of your application.
I would also provide the writer with a short list of your accomplishments that show your talent. However, you don't want them to regurgitate that list in their letter. But it will help them write the letter.
And, as other answerers have said, family friends are not that useful for recs. They obviously like you (hence friend) and they usually have little to compare you to. So unless your family friend is uniquely qualified to judge your merits, I would shy away from them for recs, as they will probably be useless to the committee.

The Guidance Counselor is most likely doing you a favor by not writing the letter. You have been out of school for a while; it doesn't seem like that long to you, but to a school administrator who is dealing with hundreds of students, a year is quite long! Because of this, she doesn't feel like she remembers enough about you to write a good recommendation. Remember that you WANT a good recommendation, not a half-hearted one written by someone who barely remembers you.Contact the admissions office of the college you want to attend and explain the situation. Ask if a recommendation letter from someone who knows you well would work instead of the guidance counselor. Usually they will accept a recommendation letter written by an employer or supervisor instead, which is good for you (especially if you've made a good impression on your supervisor!).
