google is you friend - but unless an attorney that represents you vets the letter, it will likely not stand up in court when you need it most.

and if you are going to have a lawyer vet it, you might as well have him write it because he will have to rewrite the one you give him anyway.

And you should have it signed in the presence of two witnesses, who should give their addresses and occupations.

Most courts have pre-printed forms that cover just about everything. Call the court clerk and ask if they have such a form. If not ask if they have a family law clinic (some courts have these in place to assist people who don't have attorneys)...If you and your husband are amicably divorcing ask an attorney to draw up a Stipulation for Dissolution (it's an agreeement between you and the husband that states your divorcing and outlines the terms of the divorce) good luck. If you don't want to deal with an attorney, see if there is a legal aid clinic in your area...they do low cost or free assistance for these types of things.
