You can create a legal affidavit by just writing down what you want to say and having it notarized (which means that you swear that what you wrote was true to a Notary Public who then stamps your document as having been legally sworn to).

As to what you should write...just describe some typical situations you've been in with the kids and their dads and how the dads handled those situations and the effect it had on the kids. Also, if you've witnessed any situations involving the moms that would indicate that they'd be an unfit, or just less desirable, custodian for the kids, then you could include that in your affidavit, too.
