I've usually not had to write one, either. . . if anything it's "yes" in the department office or to my advisor for a TA position, and it's usually not even that for an RA position -- just assumed consent since I'm not protesting getting paid for what I do anyway. :)

I think the 1-2 I've had to actually have something in writing for were just the "I accept this offer" sort and then sign it when I asked about that. Whoever's paying you quite likely just wants it on paper that you agree to do whatever you're getting paid for, so there's no real requirement other than some form of "I understand whatever you said my responsibilities are and I agree to do them". You can always ask the "is this all you need?" question when you give it to whomever if you're not quite sure.
