First of all, for regular retail work, especially part-time, you don't need a resume or cover letter. Just fill out the standard application Wal-mart provides.One section of the application will have days and times for you to list that you are "available to work". As far as working at different locations, an application for a major chain probably has a section that asks that too. If not, you can tell the hiring manager at any location, or just apply at all four.
But if you really want to write a professional cover letter, you need to observe all the rules of punctuation and grammar. Don't write in all lowercase letters (like you did in the details of your question) and don't use internet abbreviations (such as "HR Manager"), everything should be spelled out.
The best format for a cover letter is to look at the requirements for the job and one-by-one indicate why you have each of the skills that would make you a perfect candidate for hiring, citing information from your background wherever pertinent.
Address it "To Whom it May Concern" if you don't know the name OR gender of the person to whom your letter is addressed.
