First off, I've included a link to a sample in MS Word to get you started. It's from a human resources website and will get you set up to start. I've also included a link to a page with multiple formats and links to samples and templates for further development.In terms of the "how to" portion of your question, your letter will depend largely on your skills, your goals, and how these mesh with the company.
Step one: Research. If the company has a website, read it! Look over their mission statements, advertising, reports of earnings, goals, and any files on personnel. I'd also recommend checking out the company with a full search online to see customer satisfaction and gauge the company's relative reputation in the market. From this, you'll want to pull out key words, phrases, even the way that they select the font on their website so that you can incorporate this into your letter of interest.
Step two: Organizing. Make a list that builds on your resume. If you haven't already done so, make a "master list" of jobs, skills, awards, training, and accomplishments. (By the way, numbers are great for this step. Money made, time worked, or percentages will all give your reader something that feels concrete when you explain it) Your goal in this step is to try and pull together and match any of your qualifications that are in line with what you read about in Step one. If the company says that they "value innovative leadership" and you once led a project on building a solar-powered robot, you'll want to match the right skills to their goals.
Step three: Drafting. Give the letter a try! Before you submit it, have a friend (or nine) read it over and give you feedback. Remember that the letter should correspond strongly with your resume and will need to be clearly written for this purpose.
And throughout, always remember this mantra:
1. Here's what I've *done*
2. Here's what I'm *doing*
3. Here's what I *can do* FOR YOU.
Remember, you're are marketing yourself as the answer to their needs. While you may need the job, you have to sell yourself as the answer to their prayers!

I would start out with 'Dear Judge"But when you write this letter please do not write ANYTHING that you dont want your mom to read. The judge will probably show this letter to your mom's attorney. While I understand that you dont want to live there you dont want you to say or do anything that would permanently damage your relationship with her. I am not suggesting that you do do anything other than tell the truth but (for example) it is not fair to state that your mom smacked you in the mouth if you dont state that you called her a few cusswords before she did it.
Saying that she smacked you in the mouth makes her look like an abuser who beats her kids, while saying that you cussed her out before she did it makes her seem like a stressed person who lost her temper while disciplining you.
Both will convey the tension in the house but it wont paint your mom as worse than she is since I am sure that you love both of your parents but just prefer to live with one of them over the other.

Also see if the judge could apoint you a guardian ad litem (I think that they are lawyers who represent the interest of the child in court cases)
