Im not sure but i think you are on the right track. keep it short and simple. state that you want to return. & why.
and if youre not sure state in the letter and ask what is it that i must do to be reinstated.

Then surely they will go from there.

or when you go to the next meeting just call an elder to the side and ask them I want to write a letter to be reinstated , how do i go about doing it?
