You should start with a general outline of when you began with the company and when you first began working with and/or for this particular manager. I would also interview other workers & ask for their input. If they have any suggestions & complaints request their permission to quote them. Then go into greater detail of particuliar circumstances when this manager exhibited poor performance. This could be in employee relations, company policies, employee preferencial treatment or neglect. You should include everything you feel is poor conduct for a manager. Have a respected co-worker or two look it over and get their opinion(s) before you submit it. I wish you all the best.

Yes an effective letter should start with an introduction to the situation, followed by specific examples of the conduct you are reporting. I suggest you keep personal opinion to the minimum, and let the facts speak for themselves.
