You want to start a cover letter off by stating your interest in the industry you are applying for.Next I generally cover my past experience and how it relates to the position I am applying for.
I.E. The experience that I have gained working in customer service, as well as my previous construction experience has been preparing me for the opportunity to be able to manage a business and work independently, in order to be successful in high performance demanding jobs such as yours.
Remember not to make your cover letter more than a page long.

Then tell about your experiences (even if you have to lie) just make it sound realistic. stretch the truth, dont go overboard. if you babysat when younger or what not call yourself a home aid or something

I've been teaching medical front office and medical billing and coding for six years. I will tell you what I tell my students.First of all, work on your resume. If you are new to the medical field, focus on your studies and all transferable skills from other jobs such as being self motivated, providing excellent customer service and being good at teamwork. If you have ANY medical experience even if years ago, include it.
Right now I am getting a lot of students who are going into the medical field because they want a job. Even if this is why you went through the program emphasize that this is a CAREER for you. Many of my graduates I would not hire because they don't have the proper attitude of service to patients--they just want a job.
Before you write your cover letter, research the office's website if you know the name of the medical facility. Some of the ads only have you write back to an anonymous email account. Do your research and mention what you liked and found most interesting about the practice. What surprised you, excited you? In your cover letter also mention their needs and give specific examples of how you are a good match for the position.
If you get an interview, make sure you send a thank you letter. I was an office manager for a primary office for five years and was astounded at how many applicants neglected to do this important task.
Here are some possible first sentences depending upon whether you are front or back office assistant:
As cardiology is my favorite specialty I am excited to submit my resume for your consideration.
I love children and all the photos of the babies born due to your successful fertility clinic inspired me to apply..,
I greatly enjoy getting the doctor all the money to which he or she is entitled as quickly as possible and am interested in your internship position available as an insurance biller.
I love helping patients getting the procedures they need and am therefore submitting my resume for the insurance authorization position you have available.
Play around with the words. Hopefully this gives you some ideas on how to personalize things. Don't be afraid to have two or three resumes depending upon the position you are applying for.
