Google resignation letters. There are templates for basic ones all over the web. Keep it straight forward and if you hated your job, don't say anything negative. Never burn your bridges.

Two weeks notice is a standard. However, seeing as it is perfectly legal to be fired and told to leave, without severance pay, you can also up and quit without notice.

A main reason for two weeks notice is that you want your old employer to give you a reasonable reference. Also, your new employer will consider your leaving the old firm without notice as somewhat farty.
