I just recently had the same experience for graduate school. Teachers consider it a compliment to be asked for a recommendation. Yes, it takes a little time, but it is part of the job, and there is nothing they would rather do than to tell someone what a good job they did on you. One of my professors said, "Never apologize for asking for a letter of recommendation. I'm glad to do it." The only advice I have for you is to give them plenty of time before any deadlines. start by mentioning your name, your qualifications, and how you know the person (this should be very brief - the letter isn't about you, just say enough so they know you are qualified to give an opinion)Talk about qualities that person has that would make them right for the job (or whatever the letter is for). The other answers gave good suggestions there, I won't elaborate.
Last paragraph, breifly sum it up and say you'd recommend them for whatever. Say if they have any other questions, tell them they can contact you @ ____ and sign sincerely you

Great question. :)It can be a little nerve-wracking, only because you're asking for something - however, you'll do great, because you'll be prepared if you do the following 3 things...
(1) Ask the teacher if you can speak with them after class.
This lets the teacher know that you have something important to talk about. Most students just casually mention that they need a letter of recommendation - however, if you mention you want to see them after class, that will put them on notice - that you are wanting to talk to them about something serious (and they'll be relieved that it's something great, like a letter of recommendation! :)
(2) Prepare a statement outlining your academic and personal accomplishments, and hand them to your teacher when you ask to speak to them.
This makes their job much easier.
(3) Decide when you need the letter - and where you need the letter sent - prior to your meeting with the teacher.
Have all the details ready in advance.
You're going to do great - no need to be nervous! Teachers do this all the time, but appreciate it when students are prepared to submit the necessary information for them to write letters.

There's a big difference between a well-written statement of purpose and a convincing one - even though it may be written well, he doesn't think it will convince a grad program you're ready to do research. He told you he doesn't think it's very good - chances are good he'll be willing to help you make it better. Take him up on his offer. He knows better than you, after all - he's been reading these for years. If he wasn't going to say anything positive, he wouldn't have agreed to write the letter. I've got a few undergrads working with myself and my graduate adviser, and I know he had to tell one of them he simply couldn't write him a good letter as a result of the (lack) of work this student has done.
