Good start but definitely indicate that you have been told by 'state your father's name' and then add 'my father' that you may have a shadowing opportunity available for which I would like to make myself available. Phrasing can show confidence and commitment. Doctors don't like to waste their time on students who have neither of these attributes. It is a long hard regimen of study. Be strong in your wording. Close stronger than 'do you have'. For instance, say that you will call his office in a few days for an appointment to discuss this possibility.
Thank him for his time and attention to your letter. Sign off with a Yours Truly.
Remember to spell-check and have a person whose grammar you trust read it for you to make sure that it is cohesive and direct.

Write the address of the institution or foundation on the r top right hand side of the paper along with the date at the end. Then, use four to seven spaces before you write your own address.
After that, you write: To whoever it may concern: or Dear Sir or Madam: and then you start your letter. I would say to describe who you are and what type of information you want. After that, close it by useing Sincerely, Yours sincerely, or Yours Truly followed by a comma. You would then want to write your signature in cursive and then under it type your name.
Just a tip, read it over to make sure you spelled everything correctly and that the puctuation and grammar are correct.
