Rebecca's answer is the only one that is correct. Do not, ever, write "Dear Sir/Madam". Write "Dear Sir or Madam", and don't forget to write "Yours faithfully" as the complimentary close. "Dear Sir/Madam" gives the impression that you don't really care whom you are writing to, "and it's all a bit of a bore, isn't it?". If you want someone to do something for you, then you should be polite and appear to care, even if it's only a little bit.

Well, where do I start? If you are going to write such a formal letter you need Microsoft Office on your computer. I don't mean to be mean spirited, but your spelling will need to be checked and your punctuation. Letters are NOT the same as pm boxes at Yahoo. They are not forgiving and they are representative of you and give others their first impression of who you are. Get yourself a professional letter writer or at least an English major to help out.
