If you have a problem with the item. Take it back to the supermarket and show them. you won't be the first one to do this. I had some bad apples, they looked good on the out side but not in the middle. after I tried the second one. which was the same I soon took them back and they gave me some more.

Okay... put your address in the top left hand corner with the date below that, then on the right side of the paper on the line below the date write 'Dear Sir/Madame' or if you want to put something else you can but keep it formal! here is a link that you can look at (the green is an explication of what to do)

what about sending the letter giving the full company name and address and underneath - For the Attention of Mr XXXX

Then start the letter Dear Sirs and finish Yours Faithfully.

at the bottom of the copies NOT the original put bcc and then list the names, organisations of the people you are sending copies to
