"To" or "Attention", Editor. This is sufficient, as you've already entered the address. His name would be better, XXX XXXX, Editor

If referencing a prior correspondence, you could use,
RE: Letter dated, xx,xx,xxxx Center this.

Ending?
"Yours truly", is sufficient. Not faithfully, too personal.
"Sincerely", is OK.

If this is a request, "Thank you", or "Thanking you in advance". That's all.

First, if someone here does this it violates privacy act laws because your parents or guardians are the only ones that can legally write this.

Second, we do not know you. We don't know your character, you as a student and frankly you doing this is not very wise.

Talk to your parents or guardian and ask the mmto write. The school should not accept something from off the internet.
