It's not really clear what you're asking for, but I'll take a guess.

You're wanting to be able to e-mail your customers a form that they will fill out and send back to you and it will automatically be plugged into your database?

I believe Adobe Professional has the function to create forms that folks fill out that can collect and compile all the information.

I think what you really want though is a web page that people could go to to fill in this info. When web pages collect info, they collect it in a database. You could then connect that database to your database and import the information. You could use a survey maker website to create a "survey" and the survey questions would be the info you want to collect from your customers.

Oh wait... I just realized that you're asking for a template to e-mail out to folks that would just talk about your intent to collect this info. I think the first two options I suggested are better choices.

do you have the names and addresses typed into a program already? How you proceed depends on how your data is stored. If you have it in a word table format, copy it and paste it into an excel document, and save it.

Then you use Word to do a mail merge - from within the document, go to tools and choose mail merge option - it will walk you through the entire process. When it asks for your database for inserting the names and addresses, select the excel file you just created.

Then it will automatically insert all the data into the letter, creating a new page for each letter which you can print.
