That will vary from school to school. I think you have two options - contact the school, explain your situation, and ask them what you should do, or attach a cover letter to the old letter and explain that the contact info is not longer valid. Whatever the coach said about you is still true. I guess there is a third option - find someone else that can write you a recommendation.
