You're not required to fill the entire page. Use the first paragraph to say which job you're interested in. In the next paragraph (or two), point out the reasons you're the right person for the job. In the last paragraph, thank them for their consideration.

And make sure that cover letter (and your resume) are perfect: no typos, misspellings or misused words.

I would call rather than write.

You could mail or email them but I would recommend an actual call since it is more personal and more difficult for them to overlook your email or blow you off.

If you left on good terms there is a better chance you can get back in.
If you were a good employee who did good work and didn't have a lot of other issues going on (like a lot of drama) then your chances are better.

If you were a good worker but left without notice or left on bad terms your chances aren't very good.

If you left on bad terms or had performance issues I would stress you realized you made a mistake or are a changed person. If they say no I might ask if you let a year or so pass would they be willing to consider. If even they say they won't reconsider I might try again every 6-12 months anyway. Their need for qualified people may have them in a bind and they might say yes.

If you are on speaking or friendly terms with your former supervisor I might also ask them to put in a good word. Again, even if they say no I would keep in touch with the supervisor in case things change.

I'm HR manager at my company and we have a lot of what we call "boomerang" employees. If the employee was good and they gave us two weeks notice (or they quit without notice for a legitimate reason) usually give them a second (and sometimes third) chance.

If the employee "burned" us or had performance issues then we may make them wait anywhere from 1-3 years and have a heart to heart to see if we felt they really are a changed person.
