Also state that you make these statements under by stating that you maintain that your statements are made upon affirmation of belief and personal knowledge that certain facts and matters set forth in the document are correct and true. Sign it before a notary and you are good to go. There are forms to puchase on line as well if you so choose. Good luck with everything.
