Format - just use a general letter format. Address it to HR and your immediate boss.
Content - just state the facts: I, (name), would like to give the firm notice of my resignation. As per my contract requirements, I hereby give (number) days notice and my last day at work would be (date).
It is important to make sure you give sufficient notice or else you may have to pay the firm.
Others - usually to close the letter in a good note, do extend your gratitude to the Management for their support and guidance during your time with them.
