Sawasdee krap,I suggest you write the letter, and then let us edit it, or give you some suggestions. Keep it simple. First make a list of all the things you must include in the letter, then put them in order of how you are going to write the letter, and then finally write it.
You probably should put some contact numbers of people in authority at the school also.
Afterward, post another question and include the letter, people will help you edit it.

Short and simple. Address it to the teacher. Type it out to make it look nice. And at the end. Type your First and Last name under sincerely. And then under that, sign it. With your signature of course! Very short, to the point, yet Professional. Best of Luck to you.

I'm assuming you mean September 6th but even then you are pushing the time limit.If it's just questions that you are asking the embassy then format is not much of an issue other than you need to be very brief & concise.

First of all, the proper spelling in this situation is "principal."If you know his/her name, include it in the salutation (Dear Mr. _______). If you don't know the person, Dear Sir or Madam is correct. (But if there is any way to find out the person's name, that always makes a better impression.)
Make the letter cordial, but brief and to the point. Introduce yourself in the first paragraph. "My name is Kini Bunkport (or whatever), and I would like to put my name for the Head Boy/House Capt., in ____________ (insert the name of the school).
In the second paragraph, explain why you want to be a Head Boy/House Capt. that school, and mention any special qualities about you have that makes you a suitable student. "I'm well aware of ___________ School's fine academic reputation, and believe that I would thrive in that environment. I am an attentive and conscientious student. I have transcripts from mine previous school, and letters of recommendation from some of mine former teachers that I would be pleased to submit for your perusal.
I believe that if we could discuss this matter in person, you'd agree that I should be given consideration. I look forward to hearing from you at your earliest convenience.
Sincerely,"
