Don't worry about the title until you've written at least 4 chapters.Don't try to write too much at once. A few pages a day (or whenever the mood strikes you) will be fine.
Don't worry about punctuation, spelling, style, grammar. Just concentrate on getting the words on the page. You will revise your book several times before you're finished.
Start with a sentence or two about your idea. You don't even need to use complete sentences., just phrases. Create an outline of basic plot points (ie. characters go to school, characters interacts, characters come home ...)
Figure out who your characters are. Write up a paragraph or more about their appearance, background, hopes, dreams, etc.

Any writing teacher will tell you that you start at the beginning. :-) I do a brief summery of the story I have in mind. Then I see the major characters in the story, name them, give them a description and a biography. THEN I make an outline, remembering the rule of Three. Beginning, middle, end. Every climax needs two semi-climaxes to lead to it.Mostly I let the characters tell me what happens next. I picture them together in a situation, think what each would say or do. But I keep them under tight control. This is the hardest part. If you are writing a love story, that should be the focus. Not the protagonist's love of horses. sometimes I model characters after real people. But I am careful to change them enough to avoid them being recognized.
The second hardest part is research. It is natural to want to include all the interesting stuff you learn in your story. But the reason you research is to make your dialogue and storyline credible, not to impress your audience with your knowledge.
Write a set number of pages a day. Finish the book, then go back and cut all your very favorite paragraphs. They are almost sure to have been overwritten. That means too flowery or too dramatic. Then go back and eliminate the rest of the things that detract from the story. Every single word, every paragraph, every character's action has one purpose, to advance the story. If it doesn't do this, it belongs in another book.

A good way is to simply use the table of contents in general. But with literature, use the chapters and sections as the skeletal outline. Then put in the details or a brief summary of the chapter.Jane at the Lowood orphanage.
Jane loses her best friend.
Jane becomes a governess.
Jane meets Rochester.
Strange occurrences at Thornfield Manor.
Fire at Thornfield Manor.
etc.
