To begin your letter, either place an address heading centered at the top or block your name and address to the left. Insert two spaces to begin the date of the letter.Yes, use the complete address you've shown following the date of the letter. Insert two spaces between the date and the address, four spaces to the salutation and two more spaces before beginning the body of the text.
End with Sincerely, followed with four spaces and type your name. Apply your signature in the space between the word Sincerely and your typed name.
The objective is to balance your letter on the page of paper. If additional spacing is needed before and after the address then do so.
