You can simply say:

"As you are aware, we've seen prices of fuel, insurance, production, maintenance, and whatever rise substantially over this past year.

Due to these rising costs of doing business, we are forced to increase our prices effective ..........

We appreciate your understanding in this matter and look forward to your continuing business."

If the customer wants to argue, that's up to them .. but that doesn't mean to have to give your goods/services away just because they want you to. They have the choice to go someplace else and that's the chance you will take on any price increase ... but sometimes you have to do that to stay in business.

You need the addresses, no matter what the reason for writing it is.

There is nothing you can really do about it. Many companies have this issue whether their system is computers or actual humans. It's frustrating, but it happens. If you happen to get a notice that you think is incorrect call and ask about it, that's what customer service departments are for. Then you can also voice your concerns about the situation. Although I am sure that they will say "I'm sorry about the delay. Sometimes these things happen to cross in the mail. Be assured that this was an accident, however, and everything will be correct in your account."
